REM Minnesota's Coon Rapids program helps people with a wide range of needs including disabilities, mental health diagnoses, and other challenges live full, rich lives in their communities. Our dedicated team helps the people we serve develop daily living skills and provides as much support as they need and desire to reach their goals. Services are tailored to each person's needs and may include residential services such as Community Supportive Living homes (24/7 care), In Home Hourly Supports, and Mental Health Services.
